
New patients that have been referred to our office may worry about having to bring their medical history with them. When you call for your appointment you will be asked some basic questions and then asked to contact your referring physician to have all your records sent/faxed to our office that pertain to your diagnosis. This may include x-rays, CT scans, MRI results, laboratory test results and referring doctors’ notes.
In addition, our Financial Counselor will arrange to meet with you during one of your visits to review your benefits.
We will also need to know your entire medical history and medications that you take. Our New Patient History Form may be completed prior to your visit. You can download it and either mail or bring it with you for your appointment.
Insurance Coverage:
Understanding your insurance coverage can be difficult and frustrating. It is important for you to check with your insurance company regarding your responsibilities prior to your visit. They can tell you whether you may have co-pays or deductibles, out of pocket expenses or prior authorizations for your visits. Be sure to write the name of the person you spoke to for further reference.
At Connecticut Oncology & Hematology, we will work with you to receive full benefit from your insurance company and we will respond to any requests from your insurers for additional medical information. Our Financial Counselor will answer any questions that you may have on your insurance coverage. Please bring your insurance cards with you so we can make copies of them and bill your insurance company.
